New Patients

New Patient Registration

Thank you for choosing us for your care.   In order to prepare you for your initial visit, we need you to complete the New Patient Registration packet.

When you come to our office for your initial visit, please bring this completed set of forms with you along with a photo ID and your insurance card.


We schedule patients by appointment only and do not double book appointments to ensure that you are seen in a timely manner.  If you are unable to make a scheduled appointment, we ask that you please contact our office to reschedule as soon as possible.  We require at least 24 hours notice to change or cancel an appointment or a fee will be charged.

Financial / Insurance

We currently accept private pay,  BCBS-TX and Aetna POS.   Please contact our office to confirm that we participate in your plan prior to your initial visit.

We accept most major credit/debit cards and cash as forms of payment.  No checks are accepted.  A deposit is required to book a new patient appointment.


We value the privacy of our patients and we follow HIPAA guidelines for protecting your health information.  A copy of our HIPAA privacy policy can be viewed here.

Prescription Refills

When you need a prescription refilled, please call our office with the following information:

  • Your pharmacy name and phone number
  • Name of your prescription medication and dosage

We require 48 hours to authorize your prescription refills.
